HealthChoices Patient-Centered Medical Home Learning Network

The HealthChoices Physical Health Managed Care Organizations (MCOs) are working with Pittsburgh Regional Health Initiative (PRHI) and the Health Federation of Philadelphia (HFP) to facilitate the HealthChoices Patient-Centered Medical Home (PCMH) Learning Network—one of the provider requirements for this PCMH Program. The goals of the PCMH Learning Network are to support the PCMH providers and Physical Health MCOs in achieving the shared PCMH aims, identifying and acting on improvement strategies in response to opportunities for improvement toward the common PCMH aims and measures, and developing an internal capacity to continuously learn, adapt, and improve.

Since 2017 PRHI has coordinated the PCMH Learning Network across Pennsylvania, initially facilitating quarterly, regional learning collaborative sessions for the Southwest/Northwest and Northeast regions with the HFP facilitating the Lehigh/Capital and Southeast quarterly, regional learning collaborative sessions.

In 2025, the PCMH Learning Network replaced the regional sessions model with sessions organized by “Sprints” which are focused, concerted learning efforts to implement key interventions (manageable learning and implementation segments) within statewide groups of PCMHs working on the same focus area. Starting in 2026, the PCMH Learning Network adopted a learning session format focused on supporting PCMHs in completing one or more PDSA cycles within their chose sprint topic(s).

The Sprint Focus Areas include:

  1. Behavioral Health Integration in Primary Care for Adults and Adolescents
  2. Comprehensive Diabetes Care and Health Equity
  3. Perinatal Care in Family and Pediatric Offices
  4. The PCMH Pediatric Nursing Care (PNC) Program, with a Sprint focus on defining and strengthening the role of the PCMH-PNC case managers and developing and implementing family-centered plans of care
